For Referrers:
Thank you for considering me as part of your treatment team.
I accept Chronic Disease Management and Eating Disorder Treatment plans. Please note I do not bulk bill so, out-of-pocket costs will apply.
I work with people in person at my office in Hawthorn East with easy access to the surrounding eastern suburbs of Camberwell, Canterbury, Surrey Hills, Hawthorn, Balwyn, Box Hill, Ashburton, Mont Albert, Kew etc. I also offer telehealth appointments nationwide.
To make a referral, please obtain consent from your client/patient and then contact by email with your referral information or fax to 03 9830 0323. I will follow up with your client/patient within 3 days if contact details have been provided, or I will send you a Referral Acceptance.
Alternatively, your client/patient may contact me directly and provide me with any relevant information from you before their first appointment.
